29/3/2017 Positivity Or Denial?There comes a point when you have to take stock and say, “Things are bad.” The beginning of the economic downturn saw a tedious increase in ‘experts’ telling us that positivity would make everything better. Thinking about what is good rather than bad and focusing on the good stuff really can build new neural pathways and change your attitude for the better. But it won't help if you want to walk across the road with your eyes shut. Very quickly the suggestion that positivity can be helpful has turned into a demand. We live in a country where therapists are working with the DWP to make unemployed people go back to paid employment and where paid employment is suddenly perceived as a ‘positive health outcome.’ If you think that’s wonderful then you don’t care about evidence. It is unethical, counterproductive and has no basis in fact. It speaks volumes about the desperation that many therapists are feeling. Because you’re reading this it’s probable that you’re all too aware that things have been falling apart for some time. Many businesses can’t afford the materials and staff they need and so existing staff are working at the edges of their ability and capacity. Public services - the services we all use when we travel, get ill, expect an educated population or want our elderly relatives to have some kind of dignity – have been amputated. Things are falling apart. The most vulnerable amongst us have been telling us this for years. And now ordinary people who are not vulnerable are feeling it.
Cognitive dissonance is “the mental stress experienced by a person who simultaneously holds two or more contradictory beliefs, ideas, or values; when performing an action that contradicts existing beliefs, ideas, or values; or when confronted with new information that contradicts existing beliefs, ideas, and values.” We all experience it from time to time but many of us are finding a home in this unbearable environment. Demands for positivity have become authoritarian, insisting that we cut a smile into our face and do little dances of joy to demonstrate team spirit or something. Anyone saying, “My life is hard” is a shirker. Anyone suggesting that motivational speeches don’t put food on the table is dismissed as unenthusiastic. People who want to care for a sick child are told that they must ‘arrange alternative childcare.’ How do you discern whether positivity indicates confidence or stupidity? Intelligence, good business strategy and the ability to avoid danger involves knowing when to cut our losses. The alternative is to continue staring at the sun, borrow unsustainable amounts of money, time and good will and crash disastrously. Things are bad for most of us right now and, as adults, we need to look at facts rather than allow our prejudices and desires to rule us. Sterling is falling and the trend is downward. A 60-hour week is no longer unusual. (If you don’t work a 60 hour week that’s a good thing not an indication that you’re lazy.) Anxiety is twice as prevalent as depression. Our children are becoming more and more distressed. If you’ve read my blog for a while you know I don’t like inspirational quotes and the pretence that everything will improve if we all just pull together and think happy thoughts. Now I’m saying that if you’re an ordinary person and you’re not feeling a bit crap that it’s likely that you’re ignoring something. 21/3/2015 What Will Make You Say, 'Enough'?![]() Back in July ’14 I wrote about compulsory therapy for people with mental illness on benefits. This policy idea was not based on any research – which demonstrates that making people go to counselling is counterproductive – it was a kite-flying exercise to test public opinion: just how much cruelty are we prepared to accept? Something similar was floated last month this time seeing if the public will accept enforced major surgery. Just like the compulsory therapy business forcing obese people to have surgery perversely ignores all the research: “ . . . any mandated program should have a strong evidence base for success. Unfortunately, diet and lifestyle interventions have restricted, often transient, benefits due to biological adaptations, that act to sustain high bodyweight. It is therefore important to ask whether requiring people to participate in weight-loss programmes, despite a high likelihood of failure, is acceptable from the point of view of an individual, provider, or society. Although there is some success with pharmacological treatments for obesity, the only treatment for obesity that has been proven to be successful for substantial long-term weight loss and improved quality of life in a high proportion of people is bariatric surgery." "It is not fair to ask hardworking taxpayers to fund the benefits of people who refuse to accept the support and treatment that could help them get back to a life of work." This is the standard announcement to persuade people who pay tax (and many who don't earn enough to pay tax) to agree with policies that provably worsen medical problems. We all know people who pay tax and don’t work hard. Sometimes it’s us. Whenever I hear “Hard Working Tax Payer” I hear the Shadow echo, “Idiot” because that’s how this term is in fact being used. At the end of Downfall, a dramatisation of the last days of Hitler’s life, a boy is shown returning home to find his mother lynched by the Greifkommando. This paramilitary group murdered ‘cowards and traitors’ – old men, children, women – to create a climate of terrified compliance. In the film a civilian in a typically Bavarian hat is part of the Greifkommando: he’s not a soldier, he won’t be court martialed or shot if he doesn’t murder people, he’s just along for the pleasure of instilling terror and, if he gets the chance, killing people weaker than himself. He uses his righteous love of country as justification. Some of us succumb easier than others to this psychopathy. We can’t deal with complexity. Immigrants are bad: it used to be that immigrant simply meant non-white but now we’ve broadened the term to include people from Eastern Europe. People on benefits have always been sneered at but now the disabled are also Scroungers. We can’t cope with the fact that the highest proportion of benefits - 47% - go to pensioners so we ignore that. We’re comfortable with Them and Us where They are something to be despised and therefore so much easier to devastate. This is simply human nature. Germans didn't suddenly go bonkers in 1938, any more than Rwandans, American settlers (AKA the British) Cambodians, former Yugoslavians, Turks or any other group of people who lost their sense of humanity. Part of all of us, myself included, loves to hate. Our job is to resist that, to retain our empathy and intellect. Because apart from anything else it might be you who is cut from the herd next. Once again, our attitudes are being assessed to see just how far down that road Hard Working Tax Payers have gone. Are you happy to force someone to have major surgery? Are you? 26/2/2015 Just Doing What You're ToldPart of my training as a nurse was to spend time on a mental health ward so in 1983 I was sent to what was genuinely a bin. One of the women there had arrived as a child when the building had been a Work House. There was an elderly man with Alzheimer's, a 16-year-old young woman with alcohol problems, several young people with learning difficulties and about 20 other people with diagnoses that I knew nothing about.
Young doctors practiced electro-convulsive ‘therapy’ on the elderly woman. Every couple of days the elderly man was dragged naked and shouting through the ward and made to stand in a bowl by his bed where water was poured endlessly over him as he became increasingly distraught. The 16 year old young woman was sedated every time she challenged the staff on the basis of what she was actually feeling, which was often. In retrospect she was being punished because the staff felt threatened by her obviously splendid intellect. One of the young women with a learning difficulty was offered voluntary work at London Zoo but the staff laughed at her, saying that the idea was as stupid as she was and they prevented her going. All the women were sexually assaulted by some staff and some patients. Aged 18 and stuffed with the high moral ideals of my nursing school I imagined that this was an urgent problem. It was only because I was a gobby teenager who didn’t know better that anything got done: none of the managers and just one of the (very senior) teaching staff took me seriously. The school of nursing protected me and two members of staff were sacrificed. Nothing changed. I kept banging on about it until one of my teachers said, “What do you want? Blood?” I was flabbergasted that the alternative to dealing with grotesque abuse was perceived to be killing someone and at that point began to learn to shut up. News that staff knew about Jimmy Savile’s abuse of patients shouldn’t come as a surprise. We know that people - often very senior, generously paid people - find it easier to punish less senior people than to take whistle-blowers seriously. We know that whistle-blowers are treated with absolute contempt and ill treatment, not just by managers but also sometimes by relatives of abusers. DBS checks are a waste of time and a horrific waste of money. What actually protects vulnerable people is a culture, not of suspicion but of openness and transparency where every person from the most senior manager to the youngest student are expected to speak out about what they see. That culture is supported by a policy that has to be followed if someone alleges abuse: it's a statement on the poverty of where we are now, that a policy has to drive people towards transparency. I wasn’t the only student nurse on that ward but when I spoke to my peers about what we were seeing I was told that we were only there for 6 weeks, that it wasn’t that bad, that the staff knew what they were doing, that they didn’t want to risk a good review, that they were frightened. Just a kind of non-specific, generalised fear. As if the sky might fall on their head. Every sphere of employment is full of bullies. Often, those bullies are out of their depth and anxious because they’ve been promoted on the basis of ticking some boxes in a selection process rather than because they are actually suited to their role. How many of us would admit that, turn down the big wage and move somewhere where we might be happier? But that’s their business. What is our business – your business too – is to ensure that we safeguard people who are weaker than we are. It’s not just in healthcare; it’s in offices where you have the power to make someone’s life worse. Don’t. The energy that you will have to use to protect yourself from knowing that you have made someone live without heating, without a home, without dignity will exhaust you. The people around you need to tell you that you’re doing the right thing because they’re doing it too. If this kind of behaviour makes you feel uncomfortable, take note. Leave if you can’t change the culture. If it makes you feel powerful, if you’re just following orders, you really do need support to stop. Depression has been under the spotlight this week after Robin Williams’ suicide. It’s great that despair – lets call it what it is rather than a medicalised euphemism - and mental ill-health are finally coming out of the dingy little spare room closet for an airing and wonderful that people who are suffering depression are having their voices heard. Talking about how mental ill-health can feel shameful, that there is little parity of esteem (a nice, tight, catchphrase) between the care offered to people with physical illness and people with mental illness is temporarily refreshing. Politicians and policy makers are saying worthy things about how dreadful this and that are and how they’ll make things better.
They're being economical with the truth. People with chronic illnesses, physical and emotional, are being driven to suicide by the same ministers now saying how awful depression is, something that was recognised by the DWP back in 2012. It's only going to get worse. It’s not just people at the end of their financial tether that are killing themselves. Successful men, you are killing yourselves at a catastrophic rate. “We have a series of assumptions about suicide that are explicit and implicit, and they make a toxic mix,” Powell says. “One is that suicide is undertaken by failures: people who have no friends, who spend all their time in their room, who have something wrong with them. Are you going to talk about people close to you who might have taken their own lives if that is what others are thinking? If you say your son has taken his own life, then that means saying he’s a failure too. But when you look at the people who do this it’s quite the reverse - it’s often true that they are admired, well-loved and talented - though they might push themselves extremely hard.” Take a look at this article: "The most deadly criticism one could make of modern civilization is that apart from its man-made crises and catastrophes, is not humanly interesting. . . . In the end, such a civilization can produce only a mass man: incapable of spontaneous, self-directed activities: at best patient, docile, disciplined to monotonous work to an almost pathetic degree." Lewis Mumford, 1951 Unhappy women generally medicate and endure, unhappy men kill themselves. I’m no fan of the Good Old Days when we all lived in each others pockets and did our socialising at the communal launderette or men-only club, but when as a nation we took the decision to vote for personal prosperity people began getting more sad. Now we're reaping that whirlwind. People who bought their council houses find their adult children have nowhere to live. When we all demanded cheap washing machines it was inevitable that manufacturing was going to go abroad. When we decided to treat each other as economic units it can’t come as too much of a surprise when we are also treated not as individual people but as things that make other things function. Like a widget. Counselling falls into this trap too. Far too many counsellors join in the scroungers and strivers nonsense. Too many believe that success is a client returning to work, even in the face of a foundational belief that our job is to support the client in discovering their own meaning for their own life. For a great many people depression is a sign that your life has lost any meaning. A lot of people believe that having a high status job title, two posh cars, a big house and garden owned by the bank, and some nice clothes will mean their life is complete, but if they ever attain all that life remains just as hollow and meaningless as ever. Look to the US which is 5 or so years ahead of us. If you want that life then do nothing, it’s on its way. You may be interested to learn that the American Dream has been totally debunked: if it were true then immigrant women would be sipping champagne in a swimming pool on a Learjet. If you’re depressed take yourself seriously. As well as going to the GP and doing all the stuff you already know helps depression, think about what you want to do with your life. It may be that you want to spend more or less time with your children. You might want to spend more time awake, relaxed and communicating with your partner or you might want to get far away from them. You might regret having got on your bike like you were told to at 18, to move far away from your family, who are getting old. You may have to sell your house and move somewhere smaller (If you move out of London this won't be a problem.) you may have to take a significant wage cut. But you really are more than your job title and bank balance. You don’t need to come to counselling to discover this – though it can be helpful to get some support as you explore your fears, desires and options. But you do need to recognise that something is wrong, understand that you don’t have to do what’s expected of you – even if it’s just you who’s putting you under pressure – and then dare to think of what you genuinely want to do with the rest of your life. Campaign Against Living Miserably is a charity specifically for men under pressure. A good part of my working week is spent in other people’s offices where I've noticed that there are little signs telling people how to use a microwave, what the fridge policy (a fridge policy!) is, reminders not to steal and that in the event of a fire they must leave the building to avoid being burned to death. In one office there were 14 full colour, laminated notices telling people to come to work on time, not to pour boiling water on toddlers, to avoid spreading infectious diseases, the sink policy – yes, really - that time in the kitchen was being monitored, and that staff attendance was under review.
This in an organisation that "doesn't have the money" for a box of tissues. I'm a big fan of Health and Safety legislation, it really is necessary to tell people that hot water is hot, but Health and Safety is not the issue in an organisation that feels it necessary to watch how long individuals are spending in a kitchen. The private sector doesn’t seem to have the same issues. People there somehow know how to use a fridge without needing a policy. It's not that people who work in the private sector are any brighter than people who serve the public, far from it. It’s more that initiative is more likely to be nurtured rather than treated with suspicion and organisational vision tends to be wider than what’s happening in a microwave. Work in the public and voluntary sectors is too often reduced to a Kafkaesque model of not-work where flair and imagination are perceived as suspect and personally threatening. In the private sector innovation, ideas and positive critique are more likely to be welcomed. I've worked in places where little children bellow and cry too much while stressed workers scream nursery rhymes at them. The staff have obviously gone through training and got appropriate qualifications. The staff ratios are legal and the toilet-cleaning rota is managed very well, everything is fine on paper and that's all that matters. Sickness rates and staff turnover are costing an arm and a leg but the funders are content with the reports sent to them. Funders tell organisations what they expect to hear and then organisations repeat it back to them including, incredibly, sending them ‘case studies’ instead of saying, “No, we will not send you a case study because that would be entirely inappropriate.” Instead, organisations make case studies up. It's true that many people are put into positions of responsibility that they are not worthy of and they can have a profound effect on the people working under them. It's an irony that those people who don't feel anxious about being competent at their job, who believe they're indispensable to the organisation, are more likely to be so comfortable at work that they feel no need for personal development. Similar problems arise when a leader isn't confident enough to lead but just keeps doing what they're told rather than saying, "Why do we need to be told not to scald children? What's going on here?" Writing a fridge policy is not a good use of anyone's time. Any organisation that needs a fridge policy has far, far greater problems than manky yoghurt.

Can Compulsory Therapy Ever Work?Psychiatry has often been used as a tool of state control. People who have been inconvenient or low status or cost too much to care for have been brutalised for centuries, so last weeks 'kite flying' announcement that people who are unemployed and mentally ill may be forced to attend some kind of therapy or have their benefits stopped has precedent.
Throwing an idea out without a formed policy behind it is called 'kite flying' because the people proposing it want to see how such an idea might fly. Will we rejoice that unemployed people are being further required to perform more hoop jumping or will we boggle at what a ludicrous bit of nonsense this is? Ethically, it's a non-starter. When we begin compelling adults to have medical procedures we enter the world of ethical committees and High Courts: particularly because psychiatry has been used to abuse people compulsion in it is treated with enormous caution. This is not to say that it doesn't occur but when it's used it's almost always in a situation that is considered life threatening. People who are not sectioned but who are so depressed or anxious that they cannot work are not a threat to themselves or others. We have evidence of what happens when we put a government agency - ATOS - between a patient and their GP. The suicide rate increases and the financial cost of appeals outweighs any savings made. The emotional cost to patients and their families is often catastrophic. There's no reason to believe this new scheme will be any different. Therapeutically, counsellors know that a person who has been sent to therapy by a spouse, employer or parent is unlikely to do well. Therapy should never be a punishment or way of controlling someone, it has to be freely chosen. Yes, offenders are often compelled to attend therapy and what happens is that a majority learn the language of contrition rather than positively learning much about their motivations and their effect on victims. So we know that compelled therapy is ineffective. We can also add that if this dreadful idea was ever to be implemented it would be limited to six or so sessions, which is barely enough for someone who is mildly unhappy let alone someone with a mental health diagnosis. The waiting list - already enormous for NHS and most agency therapy - would make it unmanageable and we can guess that, just as with CBT, many of the people trained for this project would not actually be therapists at all, but technicians on a budget and under pressure. So what might the purpose of this dreadful scheme be? Would people compelled to have therapy be removed from the official numbers of the unemployed? This is what happens to people who are compelled to join other unemployment schemes so that the numbers of unemployed and particularly long term unemployed fall, on paper. If ministers wanted to help people with mental illness back to work they need to give appropriate funding to existing mental health services and reopen the services that closed because of reduced funding. But we live in a period of time when it's not quite acceptable to throw stones at the mentally ill, yet we are encouraged to pour scorn on them if they are also unemployed. If the public mood likes the idea of punishing people who are so profoundly unwell that they have resigned themselves to living on around £100 a week then this will no doubt happen. At the very best, it will offer therapy to people who have not been able to access it. At worst it will offer dreadful non-therapy from ill-trained, ill-motivated non-therapists. This idea slunk off in shame in 2009: there's no good reason why, 5 years later, it shouldn't slink off to die.

Why Would Suarez Stop?The strangest headline I’ve seen in relation to Suarez has to be:
Bruce Springsteen on Luis Suarez: 'Biting has no place in sports' All the World Cup means to me is that the streets suddenly become silent and I can enjoy an evening with the windows open. I don’t understand the hysteria and I know there’s nothing to be done about it – the urge to take sides and pour all the passion a beating heart can muster into a game is too primitive and hard wired, and there are worse ways of blowing off steam. But the Suarez hoo haa has been so big that it’s reached even me. I’ve heard other psychs discussing why and how and what Suarez can do to get help and frankly they all end up as flabbergasted as everyone else. No, an adult man biting other adult men is not an oral fixation. No, he does not have a split personality, whatever that may be. He may well have been humiliated as a child but which of us has not? Suarez is not losing control, he is absolutely in control. He expertly measures the distance between him and the person he intends to assault then makes a series of highly calculated moves in order to get his teeth around a part of them, and then he assaults them. He doesn’t run around biting people randomly, he chooses someone to assault and then he assaults them. If a man bites you while you’re waiting for a bus you call the police, the man is arrested, gets a psychiatric evaluation and a criminal record. I’m not sure why this isn’t the case with Suarez, who has done this 3 times now. Perhaps that’s where the answer lies. He’s got away with it 3 times and he’s being rewarded for it. He’s trending on twitter, he’s in all the media, he’s getting a massive amount of attention, so much so that it’s entered my football-free world where Gary Lineker is a nice man who likes crisps. Bruce Springsteen is talking about him. It’s a safe bet that more than one ad agency is swiftly creating a commercial for Suarez to sell something with ‘bite’ in it. * Biting is his trademark, he is rewarded for biting. Apparently he’s had ‘anger management’ for his previous assaults. It hasn’t worked. For it to work the person going to anger management therapy has want to stop being angry. He has to feel shame, humiliation and a pressing desire to stop acting out. Why on earth would Suarez feel that? He’s getting loads of attention and being paid for it. Why hasn’t he spent at least a night in a cell? You would, if you bit someone in the street. If the police were able to curb their excitement at having a famous footballer in their charge and treat Suarez as they would a vagrant who had assaulted someone (or like Mr Tribble up there in the picture who was held for rather longer without bail.) it might bring him down to earth with a bang and he might start taking himself seriously. The chances of that are infinitesimal. Instead, he’ll still be paid huge amounts of money that he can spend while he’s banned, and the attention won’t stop and he’ll be handled with the kid gloves that rich, famous people get. He’ll have therapy and it won’t work though it may educate him in the language of contrition. In the long run he may even stop biting but he won’t stop being violent in other ways, because there are no meaningful consequences for his violence. If you place bets, put one on Suarez biting someone again within 12 months. There was 28 months between his first and second assaults, 15 months between his second and third. It’s his trademark; people are waiting for him to do it again. Why would he stop? * Specsavers, McDonalds, Nandos and Snickers got there within 4 hours.

"I Just Don't Have The Time!"I had to sit down to listen carefully to a radio programme this morning, discussing this news. A growing number of school age children are still wearing nappies.
The assumption is that that it is underclass parents, people who can barely drag themselves from their filthy pits to tend to their almost-inhuman offspring, who are responsible for this epic neglect. Those people who called in to the radio programme to talk about their personal experience of this phenomenon felt they don’t have enough time because they're at work. The research suggests their attitude isn't limited to people who call radio programmes. They work all week and on their days off they don’t have the energy to commit to this very basic task. They can’t bear the thought of having to clean up the inevitable and quite normal mistakes that occur during toilet training so they don’t bother. Helping their child move from an infant stage to that of an appropriately independent and capable child takes a couple of weeks, and these parents can't spare that time. Quite obviously, the majority of parents from every background manage to toilet train their children. I’d propose that a parent who cannot manage to care for their children should have Social Service involvement whatever their background – the excuse that you’re too tired after work to bother with your own children is preposterous. There are any number of employers who would happily work their employees into the ground and resent any workplace legislation that gets in the way of making a profit. I’m suggesting that if an employee feels under such profound pressure that they cannot take time off work to toilet train their child then something is dangerously out of balance. Dangerously. Even the DWP doesn’t insist that a lone parent work until their youngest child is 7 years old. But there’s so much greater status in being employed rather than being a single parent on the dole. At what point does caring about your status become more important than caring for your child?

"Remember that you are an Englishman, and have consequently won first prize in the lottery of life"*![]() Happy St George's Day, a day to appreciate the many, many benefits we have as people living in a first-world economy. The World Service broadcast a particularly interesting piece last night, on how France Must Change. High unemployment, a State-bound economy in the doldrums, strikes, laughable working hours, and now – get this! – legislation that prevents employees from responding to work emails after 6pm! What an outlandish and economically naive country. Strange then, that France is ahead of our own Hard Working, endlessly striving, entrepreneurial economy, second only to Germany in Europe and fifth in the world. In a survey out today comes the news that “Britain has the lowest quality of life of 9 major European countries." France has the highest quality of life. The weather has something to do with it as does our naturally cynical nature – we expect to be treated badly and so it comes as no surprise when we’re treated badly – but here’s a chilling analysis: “We may still be enjoying the fourth highest household income in Europe, but the high cost of living means we are living to work.” Person Centred counselling has a useful principle called the Locus of Evaluation. It proposes that when we’re allowed to remain in touch with how we genuinely feel we can make good choices; good for ourselves and good for the people around us. This is called The Internal Locus of Evaluation. The External Locus of Evaluation develops when we’re told how we must feel and criticised for feeling differently from the people around us. We have to ignore our genuine feelings to continue to be valued. People who function from an External Locus of Evaluation continue to look to people in authority to decide how they must feel and often feel absolutely dreadful, even if they determinedly continue to believe they are content with the way things are. You can read up on the theory here. On Sunday the Mail felt it would be profitable to send an undercover reporter to a food bank and, despite noting that the reporter was asked lots of questions about his circumstances, recount that he walked away with £40 worth of groceries, unquestioned. In the past that would simply have added to the sneering disgust of the nation but this time it resulted in £35,000 in donations to the Trussell Trust. Times and nations are not going to change much. Against all the evidence, as a country we’re going to continue believing that the French are lazy and on the verge of economic collapse whilst we lead the world in toughness and fiscal wisdom. But, as the Daily Mail incident demonstrates, individuals are now more than ever capable of effecting change. I’d propose that the conditions that bring individuals to the point of collapse are those that we are experiencing now. We’re being told one thing – that food banks are stupidly supporting evil people to lie around laughing at the Hard Working Tax Payer – when we know something very different – that food banks are a symptom of an unequal society, that the more unequal the society the more miserable society becomes and that we have the lowest productivity in the whole of the G7. Social media is a tool that individuals use to subvert propaganda, but attending to the messages that we constantly berate ourselves with is another, more difficult matter. It can be profoundly sad and even disturbing to understand that many of the beliefs we hold dear are nothing but empty words, to realise that the way we've lived to this point has been largely meaningless and for someone else's benefit. It takes time to understand how it happens at all, but if you find yourself wondering about this kind of thing then you're ready to explore it. There's a lot to cherish in the English (and British) way of being, we'd be foolish to reject all of it, and there's a lot that needs to be examined too. *Written by Cecil Rhodes, who left England at the age of 9.

High Status, High Income, Dead.![]() 4 senior bankers have killed themselves in the last week. As a Bloomberg article says, Though the reasons are not clear in either case, the coincident deaths will feed the discussion of excessive stress levels in the financial industry, not just for the young interns working 100-hour weeks but also for accomplished executives. Stress-related resignations, heart attacks and suicides may be par for the course in a high-octane, risky businesses, but the public does not really want finance to be one of these: Its money is at stake. That sounds callous but it’s what all employers, including yours, are ultimately concerned about.
